Susumu Nakoshi, a thirty-four-year-old who has walked away from his life to live in his car between a homeless park and a luxury hotel, agrees to let medical student Manabu Ito drill a hole in his skull. After the trepanation, Nakoshi's left eye starts showing him a warped figure inside each person he meets—what Ito calls their homunculus. Drawn into the strangers behind these visions, Nakoshi is forced back toward the past he was hiding from.
