Kenzō Tenma, a Japanese neurosurgeon at a Düsseldorf hospital, defies his superiors to save a young boy named Johan Liebert over the city's mayor — a choice that costs Tenma his career. Nine years later, Johan resurfaces at the center of a string of serial murders, and Tenma — now the prime suspect — sets out to track down the patient he once saved and stop the killings.
