The twelve Gold Saints are gone, sacrificed in the previous OVA to bring down the Wailing Wall, and Seiya and the surviving Bronze Saints push on into Elysion, the deepest level of the Underworld, to finish their fight against Hades. Standing in their way are Hades's two most powerful servants, the twin gods Hypnos and Thanatos, who together hold power over life and death itself. This OVA continues directly from Saint Seiya: The Hades Chapter - Inferno, closing out the Hades arc that began there.
