Taki Tachibana, a high school boy in Tokyo, and Mitsuha Miyamizu, a girl in the rural mountain town of Itomori, begin swapping bodies in their sleep without explanation. They manage each other's routines through phone notes and scribbled messages on their hands, stumbling through school and work in an unfamiliar life. When the swaps stop abruptly, Taki sets out to find Mitsuha in Itomori, only to learn that the town holds a secret neither of them expected.
